The Rapids Family Health Team is Committed to improving the health and overall well-being of our patients. Our volunteers work with our staff to achieve this vision. If you are interested in volunteering with our many programs, please email humanresources@rapidsfhteam.ca to request a volunteer form, and to submit your resume. Volunteers are required to provide two references, a copy of your criminal reference check (working with vulnerable persons), attend an interview with the Volunteer Coordinator, and attend an orientation session prior to participating in volunteer activities. Volunteers are an important part of our Team and we welcome your interest.
Rapids Family Health Team is funded by the MOHLTC. Donations are used to enhance programs and services provided to our patients and member of the Sarnia Lambton Community. Rapids Family Health Team is a not-for-profit, registered charity and is able to issue a charitable donation receipt for any monetary or in-kind gift made to us. Donations have made a huge impact on the programs we have been able to provide to our patients and in our community.
