Accessibility and Language Services at Rapids Family Health Team

Accessibility:

Rapids Family Health Team (RFHT) is committed to meeting the accessibility needs of persons with disabilities and doing so in a timely manner, in a way that takes into consideration the accessibility needs of the individual based on the type of disability. Recognizing the dignity and independence of all employees, customers and visitors, RFHT seeks to ensure that persons with disabilities have genuine, open and unhindered access to services, facilities, employment, buildings, and premises.

Our offices are wheelchair accessible.
There is a wheelchair located in the front foyer for patients to use while at the office.

Patients and/or community members requiring more information about our commitment to accessibility may request the following documents from our clinic or find them below:

To encourage effective communication, we encourage you to speak with the team members providing you care.  If you have a question, concern, comment or suggestion about our services or your treatment related to accessibility or otherwise, we encourage you to share this with us through a variety of mechanisms including, but are not limited to, emailing Human Resources humanresources@rapidsfhteam.ca, using our suggestion box in the main lobby, or by contacting our Executive Director verbally in person or via phone at 519-339-8949.  Feedback will be accepted in accessible formats and with other communication supports, as required.

Language Services:

We are committed to providing equitable access to quality safe care for all patients. Removing barriers to communication is one of the ways in which we can achieve this goal and offering patients dignity and autonomy through the use of professional interpreters. Understanding your concerns and symptoms is essential to providing quality and safe care. We know how important it is for you to be understood by your health care team and to be able to communicate in a language that you are most comfortable with.

Professional Language Interpretation Services Update

We are currently reviewing and transitioning to a new professional language interpretation service provider. During this transition, interpretation services may be temporarily unavailable. We are working to restore services as quickly as possible and appreciate your patience and understanding.

Having access to professional interpretation also ensures:

  • You get the correct information from us and that we get the correct information from you.
  • To make sure that you clearly know your diagnosis and can take part in your treatment plan.
  • To help you understand what health care you wish to receive.

Ask for an interpreter if:

  • If you are most comfortable speaking with your health care team in a language other than English
  • You are deaf, deafened or hard of hearing

Interpreter services provided:

  • Access to trained and qualified professional interpreters
  • Over 240 languages
  • American Sign language and deaf-blind interpreters
  • Pre-scheduled in person, telephone or video interpretation services
  • Urgent telephone or video interpretation services
  • Free of charge
  • Available 24/7
  • Private and confidential service

Feedback offers us an important opportunity to learn from your experiences and assist us with improving our services.  All feedback will be collected and analyzed by the Executive Director.  If requested, you can expect to hear back in five (5) business days.
