Rapids Family Health Team has partnered with MINT Memory Clinics to offer this service to Sarnia Lambton residents associated with Rapids Family Health Team FHO Physicians. This program is offered by over 115 agencies in Ontario. The program helps to diagnose and support individuals and their families who struggling with new or ongoing memory issues. The Team approach includes primary care providers, nurses, social workers, occupational therapists, community care representatives, Alzheimer's Society support, and more. The training for this program at Rapids FHT has been funded by the United Way of Sarnia Lambton. Please contact your Primary Care Provider for a referral.
