The following is a list of Physicians affiliated with Rapids Family Health Team. Please check your Physician listing for details regarding their after-hours care. If you are unable to secure a timely appointment with your Primary Care provider for a medical episodic event the Rapids Access to Care Centre is available by calling 519-491-6188.

The Sarnia Physician Group offers an After Hours Walk-In Clinic, Monday - Friday 5 pm to 8 pm at 481 London Road.

The Sarnia Medical Group offers an After Hours Clinic by Appointment ONLY on Mondays and Fridays from 4 pm to 5 pm at 494 Christina Street North, call 519-491-3434 for an appointment.

Rapids Family Health Team currently accepts referrals from Sarnia-Lambton community-based physicians affiliated with our clinic. This means if you are a patient of one of the following physicians, you have access to any of our allied health professionals and classes we offer.
